Submitting an Invention Disclosure Process

Are you ready to complete your invention disclosure form for submission?

Download and complete the appropriate form(s).

• Invention Disclosure Form — Use for inventions that are not software or mobile apps. Only the lead inventory needs to sign.
• Preliminary Invention Assignment Form — Must be completed and signed by all inventors, including the lead inventor. Submit at the same time as or after the regular Invention Disclosure Form.
• Software Disclosure Form — Use for software inventions. All inventors must sign.
• Mobile App Disclosure Form — Use for mobile app inventions. All inventors must sign.
• Non-Software Copyright Disclosure Form — Use for copyrightable work that is not software. All inventors must sign.
• Additional Contributors Addendum to Disclosure Form — Use for inventions with more than five contributors.

Provide detailed and accurate information, which is essential for our evaluation process.
